It would be great to add patterns or be able to exclude entire directories, e.g. for caching directories.

Thanks for looking into the request. Yes, I can confirm your "if you can install a plugin/theme update through the standard WordPress admin > Plugins / Themes interface, WPT should be able to do this as well."
This only works partially, some times WPT "registers" and "finds" the plugins and themes, some times it doesn't. I haven't really found a way to reproduce it. I'll quote myself from support request 256311:

I'm having 17 plugins to update in WordPress admin. However, only 11 are shown in the WordPress Toolkit using Smart Updates. That's why I'm not getting an error, the plugins aren't showing up in Plesk indicating there is an update.

These plugins are missing, they are all paid plugins, license keys etc. are set.

    - Germanized Pro
    - WPML Media
    - WPML Multilingual CMS
    - WPML String Translation
    - WPML Translation Management
    - Yoast SEO Multilingual (for WPML)

    --------- a bit later on:

I just ran into a surprise: after adjusting the Panel.ini (like advised in https://support.plesk.com/hc/en-us/articles/360013137720-Smart-Update-fails-unable-to-correctly-clone-your-website-for-further-analysis), the custom/paid plugins are shown in WordPress Toolkit. After removing the two lines and checking for updates again, the paid plugins don't show up again.
I disabled the initial check again (added the lines) and thought the updates would show up again, but still after waiting a few minutes (cache, I thought) they don't show up again.

I thought disabling the check did the trick but maybe it was just a coincidence? But still it was interessting to see the plugin updates ...
